This is a book of philosophy that happens to be built on evidence. I am going to make large claims and then hand you the research behind them, because I have learned the hard way that a true thing said carelessly can be dismissed as easily as a false one. So where I cite, I cite conservatively. Where the science shows an association rather than a proven cause, I will say so. I would rather be unimpeachable than impressive, every time, because unimpeachable is what survives the room where someone is looking for a reason not to listen.
I should also tell you what this book is not. It is not a memoir, though I have lived most of what is in it. It is not a grievance, though I have grievances. It is not anti-anyone's institution or profession. The position of this book is narrower and harder than any of those, and I chose it on purpose: systems should be held to the standard they hold the rest of us to. That is the whole argument. Everything else is detail.
